The following information is required for enrollment:
 - Two(2) proofs of address in parent/guardian's name, such as:     
          Current Utility Bill
          Rental/Lease Agreement
          Driver's License
 - Transcript and Withdrawal Grades
 - Immunization Record
California State law requires that all incoming 7th grade students get a Pertussis booster shot, called the Tdap booster, before starting junior high. Students who have not received this shot prior to the beginning of the upcoming school year on August 18, 2016, will not be allowed to start junior high. Please click this link for additional information:Tdap Parent Information
   - Proof of date of birth (birth certificate, birth record, passport, baptismal certificate, affidavit of the parent, guardian or custodian of the minor or other legal proof)
 - Students with an IEP, in addition to the requirements above, should leave a message for David LevySchool Psychologist, at (530) 891-3066, ext. 210.

It's easy to print or view an emergency card!


It is extremely important for all students and parent to create and utilize a Student and Parent Portal account.  This account will provide you with the ability to track student attendance, course history, discipline issues, demographic information, immunization data, and current grades for your student. You can choose to receive automatic email notification of any attendance, discipline, or current grading issues. 



Parent Portal accounts are automatically set up once the online enrollment process has been completed. You should receive an email from our school containing all the information needed to open your portal.  Should you have questions or need assistance with this please call our registrar, Penny Melvin @ 891-3066 ext. 209.
